Medkits become realistic, so you have to spend 10 minutes manually patching up your wounds, then wait 4-6 weeks for your health so slowly regenerate to full.

God… This should actually be a game… Have a huge and I mean HUGE open world, and have it so if you get shot, you would be injured for a day to a week, depeding on where you were hit. It sounds like a bad idea, but the people who run around with their guns, blazing, would be at such a risk, compared to someone taking EXTREME safety.
