It’s lag. You will always see people’s crosshair trailing behind the target when you spectate them. Fixing that isn’t so easy… because this is actually what really happens according to the server. You still hit the targets due to client side hit detection / lag compensation, otherwise you would have to lead the target to compensate (like in the old days).
I am not sure if other FPS have figured out a solution for this. I can’t think of an obvious one.
The solution seems pretty ‘simple’: simply measure and communicate the information about lag. I don’t think it would be cheap though. So it’s more of a contemporary engineering problem (in the internet multiplayer fps genre,) then anything else.

The idea behind lag compensation is to compensate for lag on the player’s end, meaning that server side and client side are always going to be different.
There really isn’t any way to fix that except play at a LAN, what happens server side is what matters, and there really isn’t any way to fix it without making the game run like crap because it’s constantly readjusting everything for multiple possible player side perspectives and setups.
That is true. But a (casual) player (or a spectator) that is out of the game, does not prioritize time over accuracy. The problem is the amount of information that has to be communicated between partners, (reliable, in a short amount of time,) in order for it to be accurate. Would love to see this distinction between casual and competitive (and spectating competitive) though.
True but the thing is not everyone has the same connection

We won’t get that kind of internet unless someone breaks the laws of physics a round the world trip at lightspeed is at least 120ms and this doesn’t account for computational time.
But more on topics as the games has client-side hit detection, we will never get perfect crosshair placements while spectating - basically what @Zenity said 
My point is, you could introduce a further delay, in order for it to be more accurate (when speccing as a casual player after death or spectating competitive.)
