One point I keep seeing come up is medic trains and how easy it is for medics to tap e and boom, players back on the fight.
One change I, and others, really like is the minimal sparks charge. While it was primarily done to stop pot shots, I think it would add a more tactical decision for other medics.
At the moment, a viable tactic is to run around tapping e and getting people up in the middle of a firefight. Putting in a minimal charge time would make medics think “is it worth it, or should I secure the area first, otherwise we both die.”
