The feature for "Report Abuse" isn't working for me, so i have to turn here

Hello. I’d hate to cloud up general anymore than it normally is with an off-topic post, but this is the only place i can turn to. I have been to https://support-dirtybomb.nexon.net/hc/en-us/requests/new?ticket_form_id=61499 , and when i try to submit, the button grays out and it won’t submit. So, i apologize, but it seems this is the only way i can even report this guy. Here is EVERYTHING that page said was required-
