As far as I can tell, it has happened twice to me. I’ve gotten a random Iron loadout (Aura and Nader) I think, but neither can be found among my loadouts.
That button to Quick Open a case - Loadout show but not added.

I’ve noticed this happening to me, and a “Clear Filters” button appears on the loadouts screen, next to the other filter button. Click that, and your new card(s) should appear, at least, that’s how it’s worked for me.

Yeah I had the same thing, but it’s a filter issue. You have to change the filter to something else, then try sorting by rarity again, and it’s there.
I guess that is a bug in it’s own.
