…the .blend file doesn’t have a simple format description, but (almost) binary dumps it internal database. For reading it back, you need sophisticated code. Can be found in our source tree.
A team is working on an XML of the .blend file for that reason. Estimated time of delivery is unknown… might be weeks, might be months. Announcements of this will be at the blender.org frontpage.

Methinks converting from XML to MD3 would be somewhat easier than from binary dump, so hopefully when this arrives some coder might make a converter. Heh. I might even have a shot myself.
