Many times a server says 11/12 but when I try and connect, it says its full. But then I refresh and it still says 11/12 players on the server.
So I guess there is some delay for updating between server and lobby?

Heh, I actually saw Provok today on 2 servers at the same time in browser 
I had this happen as well. Last time was today, several times.
I thought it was a matter of spectators being recognized by the server as a normal player and ‘stealing’ a spot.
It is probably someone already connecting to the match but hasn’t updated on the roster yet.
Something related has been happening just now, but the opposite 
I refresh the list of servers, join in to one that says “11 players out of 12” and when I get in there are only 3 or 4 players. I get out, chose a different one and the same happens. Several times. I had never had that happen before.
So, it seems some servers aren’t reporting the actual number of players.
Found a couple of recording regarding the issue reported by the original poster. It seems sometimes servers don’t report the correct number of players.

This is due to there being no waiting list (oddly enough I’m working on waiting lists IRL for our software :)) to create a ‘buffer zone’ so you don’t get this last man issue. I’m pretty sure this is in the pipeline (pun intended) though. As a short term fix it should check the full status before it pops up the server details screen (with the player list) - ie just refresh the entry in the list again.
