I have the same issue, i mostly spray and pray, so… holding mouse 1, i get a delay of 1 (sometimes 2sec.) and after that the weapon starts to fire. Sometimes, the weapon fires, stops for 1 sec. and fires again. I was about to give up on the mouse 
For the grenades, i thought is my mouse, changed the usb port, reinstalled the driver, increased priority for the process, etc. This problem appeared on both US and UK servers. Same, select grenade hold mouse 1, start to count to 1, then 2, grenades starts to fly, Me: " Fk you grenade! I did not finish! :|"
One time on the US server i got the weapon misfire (pistol) with some Fov changing, pressed mouse 1, fov changed fast from 110 to 100(95) with every shot fired. Only with pistol, i was playing a FdOps, 2-3 people on the server. I remember in W:ET was some cfg like this changing fov as soon as you try to shoot never liked.
