Playing Nader, I died while firing the GL and it looked like her trigger finger seized because she kept firing while incapped. She emptied the whole thing while rolling on the ground and then as the cooldown brought additional nades, she would fire those as well.

This is a common bug that happens with all cooldown based abilities.
Same can happen with ammo packs and health packs.
