I’m using simple dot-circle crosshair and I noticed that dot in the middle is not centered properly while using ironsights, but is centered with MOA scope. Here is the example (K121, MP400, MOA):

It’s not that far from center, but it really puzzles me.

Stark sights are the best, the cross goes everywhere but the center of your screen.
had this with another game which shall remain nameless.
Changing the crosshair png from 100x100 to 101x101 fixed it.
but you know using k 121 iron sights is “spraying and praying” but it is ok on medium range
Shoot a bit and you’ll notice that the in game crosshairs are offcenter, a little bit too far to the right and a few too many pixels up.
Sadly, this is actually an issue with the iron-sights themselves – some of them are simply out of whack. The most noticeable one I can think of, off the top of my head, is the empire 9. They are SUPPOSED to be centered, but they are slightly misaligned. If you are using a crosshair overlay like that, rely on the crosshair, NOT the iron-sight. 
They did some minor fixes to some of the worst aligned iron-sights a little while ago, not sure if they have plans to do further tweaks to the ones still misaligned… I would assume they would though.

God damn it I’m already a terrible shot, I don’t need irons that are off.
not like I ever actually use irons anyway.

Always felt something is wrong with ironsight shooting, because whenever I aim heads with it, somehow I was shooting bodies or slightly off head, missing shots.

I simply rely on me holding down the fire button and screaming like Rambo when I use the Emp9 (and most guns) while using ADS.

Do different intonations of the screams affect how accurate the sprays are?

Yeah. The more people I need to kill the louder/higher pitched the scream gets. One time I was stuck in the other team’s spawn on an 8v8 server. I sounded pretty girly while mowing that particular lawn.
