Just wanted to point this out, At the moment the option to create a fireteam with someone is directly beside vote to kick them. Since I got the game on Friday I have set up numerous fireteams with success, but it has happened on a few occasions now where I have accidentily started a vote to kick someone instead of asking them to join the fireteam.
People I know have also had this experience when trying to start a fireteam with me. There is currently universal consensus amongst the people I know that it would be a good idea for the options to be moved or rearranged. fortunatly no one has been kicked through this yet but it could be very damaging to teamwork to accidentialy kick someone you wanted to work with.
