No idea what is causing this, but 3 times since the Containment War update my Fire(LMB), ADS(RMB) and Weapon 3(3 & MMB), and Interact (F) keys have completely unbound themselves. I’m not talking reset to default here; upon starting the game they’re simply no longer bound to any keys. So far its only happened to these four.

This happens to me like once a week. It started before CW update for me. It´s the binds for Primary Fire, Secondary Fire, Interact and Quick ability that disappears.
