A good start is always to take a look at existing vsay mods.
For example:

Actually you have to create a new pk3 file (a renamed zip file) starting with a letter “greatter” than “p”.
For example “voicechat.pk3”.
The files of that pk3 file have to be in the same path as the original ones in the pak0.pk3 file.
For voicechats you have to edit the following files:
/ui/wm_quickmessage.menu
/ui/wm_quickmessageAlt.menu
/scripts/wm_allies_chat.voice
/scripts/wm_axis_chat.voice
I recommend to extract those files out of pak0.pk3, create a new folder structure and put these files at the correct places in this folder structure.
Then edit the files like needed, add the soundfiles in the path that you have specified in the voice files and zip a new pack keeping this folder structure.
