How do you heavy swing with a melee weapon?

By default right click and middle mouse button.

If my memory serves me: middle mouse button, and switching to melee and right clicking.

You could just go to keybinds and figure out which button it is yourself. Some people have been having problems with keybinds unbinding for no reason.

Afaik.
The Quickmelee (default middle mousebutton) is a heavy swing.
If you equip your melee weapon (default 3) you get a fast swing (default primary mousebutton) and the same heavy swing (default secondary mousebutton). Of note is that switching to melee and performing a fast swing. Is faster than quickmelee, very important for finishing people off.
