If you bring out your defib, for example, then swap to your main weapon while you have your left mouse button pressed will not fire your weapon right away. You have to depress then press again to actually fire the weapon. I notice this when I am trying to revive someone and I am under fire. It has gotten me killed quite a few times. It seems to happen with ammo boxes and grenades as well.
