Often happens that people swap to other team and sometimes because it there is one team with 2 less players than the other.
IMO you should restrict the swap. Don¡t allow to swap to red team if red team have one player more yet.

You dont played enough execution it seems. Cause alot of times there are 2 more people on other team, that is not balanced.
