Here is exactly what the error says:
Game Error
This disc is unreadable.
- Clean the disc with a soft cloth.
- Restart the console.

I get this error sometimes before I make it to the menu screen, but 100% of the time within 5 minutes of starting the game. 95% of the time I can’t even get past the character select screen. I made it to the tutorial video once, then it instantly gave me the error again once the video finished. This is happening while the game is installed.
Things I have tried:

I figured maybe it was my Xbox. Nope, I played a game of Battlefield: BC2 and got no errors. Some the Xbox is not to blame.

I cleared my Xbox cache thinking it may have been a faulty installed patch. Nope, still get the error after downloading the patch again.

I figured maybe my initial install went bad, deleted and reinstalled. Still get the error.

I did what the error screen told me to do (clean the disc and restart Xbox). Nope, that didn’t work.

I inspected the disc, I’m no expert, but the appearance of the disc looks normal. No chips, scratches, etc.
However, I have played for about an hour or two while the game is being read straight from the disc (not installed) and it appears the error does not happen then.
