Couldn't revive team mate, "inside" the ammo crate

Somehow a team mate was incapacitated “inside” the ammo crate. Couldn’t revive, although I kept on trying.
Didn’t see where he was before dying, probably on top of it.
Although I’d think the map is irrelevant this was in Terminal, behind the Attackers’ MG facing the Defenders’ MG.
